Responsibilities

• Provide comprehensive primary care , including preventive care, chronic disease management, and acute visits.
• Work closely with physicians and care teams to deliver coordinated, high-quality care.
• Maintain accurate and timely documentation in the electronic medical record (EMR) .
• Promote patient education, health maintenance, and wellness initiatives.

Qualifications

• Graduate of an accredited Nurse Practitioner program (FNP preferred)
• Board Certified as a Nurse Practitioner (AANP or ANCC)
• Active or eligible South Carolina NP license and DEA license
• Experience in primary care or internal medicine preferred
• Excellent communication and collaboration skills with a patient-centered approach
