Provides healthcare and educational services for City residents by supporting vaccine administration, prenatal care, and other healthcare programs. Conducts home or telehealth visits, maintains patient documentation, and leads clerks and other medical staff operations. ESSENTIAL DUTIES & RESPONSIBILITIES The intent of this job description is to provide a representative summary of the major duties and responsibilities performed by employees in this job. Employees may be requested to perform job-related tasks other than those specifically presented in this description. * Provides home and telehealth visits to first time mothers * Utilizes motivational interviewing, develops care plans, refers clients to additional services, and monitors effectiveness of care plan * Assesses client/child health, provides client education, coordinates care with other providers, and addresses social determinants of health * Maintains accurate records and documentation of home and telehealth visits * Participates in nursing and City training activities, as required * Reviews nursing literature and evidence-based practices on maternal and child health * Attends community coalition meetings, participates in public health emergency response, and attends outreach events * Integrates all public health, maternal and child health, nutrition and allied health services to ensure good prenatal, postpartum, newborn, infant and well childcare during the first 2 years of life * Conducts nursing home health services for maternal child health services (assures prenatal care compliance and newborn, well childcare, growth and development) * Links all appropriate health care services from primary care, well child, Immunizations, WIC (Women, Infants and Children), social services with the department and with other agencies for appropriate prenatal, postpartum, newborn well child, and positive child growth and development * Assists medical team in tracking and follow-up care for both mother and child * Assures newborn has all well child services and provides health education and support for the first 2 years of life (includes observation between mother and child) * Conducts quality assurance for patient care in the home setting * Complies with a City of Laredo policies and procedures for nursing, public health services and administrative procedures. Proof of TB Test/Screening will be required to be presented prior to employment Required Licenses or Certifications * State of Texas Registered Nurse License is required. * CPR Certification is required. Copies of certifications must be attached to application Valid Texas Driver License. If applicant holds an out-of-state license, a State of Texas Driver License must be obtained within 6 months of employment.
