Residential Case Manager YMCA Safe Place Services

The incumbent develops an individualized case plan for each youth in emergency shelter and directs, provides, and/or coordinates the provision of counseling, mediation, short-term conflict resolution, case management, medical, educational services, and home placement for youth in Shelter House. The average short-term (1 week to 3 weeks) case load will up to 10 residents and 10 aftercare families for a total of 225 youth & families per year.
ESSENTIAL RESPONSIBILITIES:

- Demonstrates a commitment to the charitable work of the YMCA and accepts a leadership role in the activities and events of the YMCA Annual Campaign.
- Challenges people to accept and demonstrate core values of the YMCA of honesty, respect, responsibility and caring.
- Provide crisis intervention, case management, and mediation services through the crisis phone line, crisis walk-in service, community-based referrals, and Safe Place program.
- Facilitate the referral process for all potential clients.
- Provide intake, case assessment and disposition, case management, “Family Team Conferencing” Mediation, and aftercare referral services to youth and families seeking services at Shelter House.
- Provide liaison and advocacy with appropriate mental health centers, courts, physicians, school systems, CHFS/DCBS, and other community agencies.
- Insures that medical and educational needs of all shelter youth are addressed.
- Conducts follow up contacts with youth and families, conducting an outcome measure survey with each family, and enters data into database system (ServicePoint).
- Assist Youth Work Staff in assuring that youth and family needs are being addressed on a day-to-day basis during the shelter stay.
- Develop effective working relationships with Senior Youth Workers, Part-Time Youth Workers, and all supervisory personnel, which contribute to collaborative decision-making and mutual accountability.
- Insures proper documentation and protection of confidential information about youth, services provided to youth and families, and contacts with other agency’s personnel about care of the youth.
- Attends and participates in meetings, supervision, and weekly case reviews.
- Assists Youth Work staff in direct supervision of residents.
- Assists in emergency transportation of youth.
- Maintain consistent and timely records, including relevant reports to meet funding and licensing requirements.
- Assist in the preparation for monitoring visits by funding and licensing sources.
- Participate in the hiring process and orientation of new staff when appropriate.
- Participate in staff and program development.
- Maintains shared case management offices and rooms in a clean and orderly condition.
- Insures prudent use of resources, assists in revenue development, and participates in the internal fundraising campaign.
- Supervises 1 - 2 students completing practicum assignments.
POSITION REQUIREMENTS:
- Master’s degree in Social Work preferred
- Two years Case Management experience
- Five years experience working with at-risk youth
- Minimum of 21 years of age.
- Valid Driver’s License and able to pass insurance requirements to transport youth in company vehicle

STATUS: Full Time; Exempt
STARTING SALARY: $43,888+ (Based on experience)
Education Requirements (Any)
Master’s degree in Social Work preferred.
Certification Requirements (Any)
Valid Driver’s License and able to pass insurance requirements to transport youth in company vehicle
Two years Case Management experience
Five years experience working with at-risk youth
