POSITION SUMMARY
Wellness Navigators will provide pre? and post?release support to individuals diagnosed with substance use disorders, collaborating with the Wellness Team to facilitate psychoeducational groups, conduct assessments, and support individuals with individualized treatment planning and recovery goals.
SERVICE RECEIVED
Receives direction and oversight from the Assistant Superintendent and Director of Behavioral Health and Treatment.
MAJOR DUTIES
- Evaluate and assess individuals for post?release treatment programs, including phone screenings and interventions.
- Collaborate with the Treatment Coordinator, Reentry teams, and other Department staff for shared program needs and referrals.
- Provide treatment and clinical case management, addressing legal, medical, and mental health needs.
- Conduct biopsychosocial assessments, obtain screenings, and develop treatment and discharge plans.
- Manage individual program progress and compliance with established goals and treatment plans.
- Provide support and services to individuals classified and released to SOCSD halfway house partners.
POSITION REQUIREMENTS
- Minimum of 1?3 years of full?time experience working with individuals living with substance use disorders in a criminal justice setting.
- CADC, LADC, or Recovery Coach Certificate required; Bachelors degree in a related field preferred.
- Experience with evidence?based treatment in criminal justice settings, including medication treatment for substance use disorders.
- Knowledge of community?based resources for housing, employment, and health care.
- Knowledge of criminal justice system and related policies and procedures.
- Ability to assess and evaluate individuals for treatment and program needs.
- Ability to communicate and collaborate effectively with individuals, families, and the broader community.
- Good oral and written communication skills.
- Experience in using computers and technology, including Microsoft Office tools.
- Ability to maintain accurate documentation and data.

- Engage in treatment education and group workshops.
- Observe and evaluate patients to create outcome measurements.
- Maintain professional relationships in the community.
- Follow medical information security guidelines and documentation.
- Manage patient records accurately and confidentially.

- Deliver community outreach event presentations.
- Monitor patient safety protocols.
- Ensure documentation system accuracy.

- Participate in cultural competency training.
- Practice and maintain scope of practice.
- Adapt to change while performing assigned tasks efficiently.
- Maintain a professional and courteous demeanor.
